Combining health and social care services in Enfield

Enfield Clinical Commissioning Group is running two events regarding plans to combine the delivery of health services and social care services.  They would like to draw on the experiences of people who

  • sharing health and care mar15 croppedAre 65 years old or older (or care for someone that is)
  • Use health and/or social care services regularly (GP, Social Care, Occupational Services, District Services etc.)
  • Have attended Accident and Emergency Services recently; or
  • Have been admitted to hospital recently.

  • Monday 30 March 2015, 1pm to 4pm
    Enfield Civic Centre, Conference Room
    Sliver Street, Enfield EN1 3XA
